Pursuant to and for the purposes of Article 13 of the Regulation (EU) 2016/679 on the protection of individuals with regard to the processing of personal data, as well as on the free circulation of data (General Data Protection Regulation, hereinafter "GDPR") Recordati Rare Diseases SARL with its headquarters 70 avenue du Général de Gaulle, 92800 Puteaux, France, Data Controller of the treatment, informs you that the personal data you will provide with the attached “Personal Information Form” (Name, Surname, emails addresses, phone numbers, other contact details) will be treated in full compliance with the GDPR, the applicable local legislation and any other applicable measures adopted by the Authority for the protection of personal data, for the following purposes:
Managing the communication with you in regards to products, medical and research related information and informing you about future projects and services provided by Recordati Rare Diseases.
The provision of your personal data and their processing for the purposes indicated above, is optional; your refusal will prohibit Recordati Rare Diseases SARL to proceed in providing you with the above-mentioned services, materials and information or to satisfy your eventual requests.
The data collected will be retained for a maximum period of five (5) years. The Data received by the Data Controller may be communicated, for the purposes mentioned above, to companies of the Recordati Group and to third-party companies (providers selected from companies with proven reliability, operating in the consulting and/or scientific and professional training sectors). The personal data collected will be used within the European Union only.
As Data Subject, you may exercise the rights referred to in Articles 15 et seq. of the GDPR (to access personal data, to request rectification, cancellation or limitation or to oppose their processing, as well as to withdraw consent at any time). You will also have the right to lodge a complaint with a supervisory authority.
